What you'll do
- Monitor and assess individual mental health/wellness status
- Develop a therapeutic relationship with individuals
- Educate individual and/or family on diagnosis/symptom recognition
- Conduct care planning meetings with all relevant service providers
- Act as a linkage to community services
- Ensure that a person-centered planning process is utilized
What it takes
- Bachelor’s degree in human services or related field
- Minimum two years’ experience in behavioral health
- One year’s experience in substance use setting preferred
- Knowledge of mental illness diagnosis and wellness strategies
- Good organizational and time management skills
- Good oral/written communication skills
