What you'll do
- Collaborate with the ICM team to identify and address healthcare needs for vulnerable clients.
- Assist clients in navigating healthcare systems, public benefits, and educational institutions.
- Develop and distribute personalized self-sufficiency guides and service plans.
- Maintain accurate client documentation in the ETO database.
- Provide extended orientation on health and social services to promote client independence.
What it takes
- 3-5 years of case management experience with similar populations.
- Relevant degree in social work, public health, or equivalent lived/professional experience.
- Professional proficiency in English; proficiency in Dari and/or Pashto strongly preferred.
- Driver’s license, reliable transportation, and valid insurance.
- Proven ability to work effectively in a multi-cultural environment.
