What you'll do

  • Complete assessments and Individual Service Plans (ISP) per DDS/MRC standards.
  • Document daily progress notes in accordance with policy and billing requirements.
  • Coordinate service delivery including housing assistance and SSI advocacy.
  • Maintain positive relationships with individuals, families, and outside service providers.
  • Identify and schedule necessary appointments and service linkages for clients.

What it takes

  • Bachelor’s degree or equivalent experience.
  • Previous experience working with intellectually disabled or brain-injured adults.
  • Ability to manage a client caseload in an outreach/community setting.
  • Valid driver’s license and a reliable vehicle.
  • Ability to travel to Lawrence, Lowell, Burlington, and Arlington for community-based work.
