What you'll do
- Complete assessments and Individual Service Plans (ISP) per DDS/MRC standards.
- Document daily progress notes in accordance with policy and billing requirements.
- Coordinate service delivery including housing assistance and SSI advocacy.
- Maintain positive relationships with individuals, families, and outside service providers.
- Identify and schedule necessary appointments and service linkages for clients.
What it takes
- Bachelor’s degree or equivalent experience.
- Previous experience working with intellectually disabled or brain-injured adults.
- Ability to manage a client caseload in an outreach/community setting.
- Valid driver’s license and a reliable vehicle.
- Ability to travel to Lawrence, Lowell, Burlington, and Arlington for community-based work.
