The role
PATH is seeking candidates to join their Supportive Housing Services Team as a Case Manager at the Winnetka Village Senior Apartments in Winnetka, California. The role involves assisting clients in breaking the cycle of homelessness through individualized service plans and referrals to resources.
What you'll do
- Conducts intake and individualized needs assessment for all clients
- Collaborates to develop Individualized Service Plan (ISP)
