The role
The Case Manager will provide case management and supportive services to chronically homeless single adults with HIV/AIDS or co-existing serious and persistent mental illness, substance abuse, or MICA disorder.
What you'll do
- Complete program documentation and enter client services into Client Track
- Refer and connect clients to needed services
- Ensure clients are compliant with medical treatment and medication management
- Monitor clients' progress toward their goals
- Function as client liaison/client advocate
- Escort clients to appointments
What it takes
- Bachelor’s Degree
- At least 4 years’ experience in case management or related social service field
- Experience working with persons with a history of homelessness, mental illness, and/or substance abuse
- Good written & verbal communication
- Computer literacy in Microsoft Office Suite
