The role
CAMBA is seeking a Case Manager to provide case management services to 27 families formerly residing in NYC homeless shelters under the Our Space initiative at Prospect Plaza in Brooklyn, NY.
What you'll do
- Maintain professional relationships with clients and client confidentiality
- Conduct initial intake and assessment of tenants and tenants' families' needs
- Assist tenants in attaining their goals by identifying and locating community resources
- Monitor tenants' progress toward their goals via regularly scheduled visits
- Act as tenant liaison/tenant advocate with outside organizations
- Schedule and escort tenants to appointments
What it takes
- Bachelor’s degree (e.g., B.A., B.S.W.)
- Experience working with the Seriously and Persistently Mentally Ill (SPMI) population
- Ability to maintain fingerprint clearance
- Mandated Reporter with the ability to report suspected incidents/cases of abuse and neglect
- Flexible hours for occasional shift coverage until 8 pm
- Proficiency in using automated database systems
