Easterseals MORC is seeking a Case Management Coordinator for their Assertive Community Treatment (ACT) team in Southfield, MI. This role involves assessing individual needs, developing program plans, and providing ongoing support, rehabilitation, and crisis intervention for clients and their families. What you'll do

  • Collaborate in daily team meetings and meet with the ACT psychiatrist twice weekly.

What it takes

  • Bachelor's degree in a Human Service field, Psychology, or Social Work.
  • Qualified Mental Health Professional (QMHP) status with at least one year of experience.
  • Valid Michigan driver’s license and proof of auto insurance.
  • Ability to engage families and individuals with empathy and a hopeful manner.
