The role
Easterseals MORC is hiring a Care Coordinator Assistant to monitor treatment plans and support individuals with intellectual and developmental disabilities.
What you'll do
- Monitor individual treatment and support plans
- Gather and compile personal and demographic information
- Understand rules and regulations governing care for individuals with developmental disabilities
- Attend planning meetings using person-centered principles
- Coordinate transitions into personal residences and broker services/supports
- Linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of supports and other community services/supports
What it takes
- High School diploma or equivalent
- One year experience working with individuals with intellectual and developmental disabilities
- Reliable means of transportation with proof of auto insurance and valid Michigan Driver's License
