Endeavor Health Services is seeking a Social Care Network Navigator to support participation in Western New York and Finger Lakes Region SCNs through the New York State 1115 Waiver. The role involves conducting SDoH screenings, providing navigation services, and coordinating referrals.
What you'll do
- Conduct Social Determinants of Health (SDoH) screenings using a standardized assessment tool
- Provide person-centered navigation services to connect individuals with community resources and support services
- Meet with individuals in person, by phone, or through secure virtual platforms
- Identify barriers related to housing, food insecurity, transportation, healthcare access, and other social needs
- Coordinate referrals and collaborate with healthcare providers, community organizations, and internal staff
- Maintain accurate and timely documentation in the Findhelp platform and internal systems
What it takes
- Experience in case management, care coordination, or a related social services role
- Valid driver's license with an acceptable driving record
- Reliable access to a personal vehicle with the ability to travel throughout Western and Central New York
- Proficiency with Microsoft Office applications
- Bachelor's degree in Social Work, Human Services, Public Health, Psychology or a related field (all levels of education will be considered)
- Experience using standardized assessment tools
