The role

The Residence Case Manager provides individualized case management services to help residents transition into self-sufficiency and independent living. This role involves conducting intake interviews, developing housing goal plans, and providing ongoing support and mentoring to residents. The position requires effective documentation, collaboration with outside service providers, and acting as an Agency Administrator for the HMIS database. The manager plays a vital role in the YMCA's mission by fostering an environment that promotes personal success and community integration.
