The role

The Registered Practical Nurse (RPN) at UHN's Transitional Care Program will provide compassionate, collaborative, and competent patient care within a multi-disciplinary team. The role involves planning and evaluating nursing care, participating in inter-professional rounds, and mentoring students and new staff. The position requires a commitment to patient safety and adherence to the College of Nurses of Ontario standards of practice.
