The role

The PoWER Lead Community Health Worker (CHW) will support the Kentucky Rural Health Transformation Program by serving as a key member of the PoWER Program Team and providing leadership, technical assistance, and implementation support to pilot site CHWs.

What you'll do

  • Serve as primary liaison between pilot site-based PoWER CHWs and the PoWER Program Team.
  • Provide leadership in developing and improving the PoWER CHW model.
  • Partner with program team members to support coordinated design, implementation, and achievement of program outcomes.
  • Coordinate with RHT CHW program leadership on aligning PoWER CHW expectations with perinatal training/certification direction.
  • Collect, review, and analyze data from PoWER sites as appropriate.
  • Collaborate with Kentucky Office of Community Health Workers and other partners.

  • Minimum five years relevant experience in a related field
