The role

The Outreach Coordinator will advance program enrollment for the Extended Case Management team by conducting targeted community outreach and building strategic partnerships. This role involves acting as a key ambassador to faith communities and public agencies to increase program visibility and ensure referral quality. The position is responsible for tracking outreach metrics, creating promotional materials, and collaborating with internal teams to meet enrollment goals. This is a limited-term role funded through September 30, 2026.
