The role

The University Health Network (UHN) is seeking a Nurse Practitioner to provide advanced clinical care within the Complex Continuing Care program at West Park Healthcare Centre. The successful candidate will work autonomously and collaboratively with interprofessional teams to diagnose, manage patient care plans, and implement evidence-informed practices. This role also involves leadership in quality improvement, staff mentorship, and participation in research initiatives to improve patient outcomes.
