Nurse Case Manager

The role

Eliot is seeking a licensed Nurse Case Manager to join their Program for Assertive Community Treatment (PACT) team. The role focuses on providing comprehensive treatment, rehabilitation, and support to adults with severe mental illness through case management and nursing interventions. The position requires outreach, medication management, and the ability to work rotating shifts to ensure 24/7 support for clients.
