The role
The Mission Case Manager implements strategies to achieve the goals for the organization and Mission Services, engaging service seekers in a collaborative planning process and providing coaching, navigation, and referrals.
What you'll do
- Engage service seekers in a collaborative planning process
- Conduct intake and ongoing assessments
- Develop, implement, and update individualized service plans
- Provide coaching, navigation, and referrals
- Coordinate service seeker support with internal teams and external partners
- Maintain regular contact with service seekers
What it takes
- Two years of college education or experience equivalency
- Minimum of 3 years’ experience in case management, coaching, navigation, intake, or direct support services
- Experience working with diverse populations
- Strong organizational skills and attention to detail
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Valid Driver’s License, a vehicle, and insurance coverage
