The role
The Medical Case Manager provides social work services under the direct supervision of the Senior Director of Behavioral Health and Supervisor of Behavioral Health. Responsibilities include case management, advocacy, HIV-related counseling, and follow-up services to patients via face to face or through telemedicine.
