The role

The Linkage Navigator serves as a vital care coordinator and case manager within a multi-disciplinary team to support whole-person health. The role involves addressing social determinants of health, managing service referrals, and facilitating access to preventative care and community resources. This position acts as a key liaison between clients, clinicians, and external providers to ensure seamless service delivery and improved patient outcomes.
