The role
Endeavor Health Services is seeking a Licensed Practical Nurse (LPN) to join their Critical Time Intervention (CTI) team, supporting individuals during key life transitions from institutional settings back into the community.
What you'll do
- Provide intensive case management and nursing support during transitions from institutional to community-based care
- Conduct health assessments, monitoring, and vital signs
- Develop and implement individualized care plans in collaboration with clients and interdisciplinary teams
- Coordinate access to medical care, behavioral health services, housing, and community resources
- Support hospital discharge planning and post-discharge stabilization
- Educate the people we serve and families on health management, medications, coping strategies, and available supports
What it takes
- Active Licensed Practical Nurse (LPN) license
- Valid driver’s license and reliable transportation
- Minimum 2 years of experience in community health nursing, behavioral health, care coordination or case management
- Strong understanding of community systems, including medical and behavioral health services, housing and supportive housing programs, social service and benefit systems
- Proficiency with Microsoft Office applications
