PATH is seeking a Lead Case Manager to join their Interim Housing team in Los Angeles to help clients break the cycle of homelessness. This role involves maintaining a direct caseload while providing administrative oversight, staff training, and contract compliance support. The successful candidate will collaborate with multidisciplinary teams to develop individualized service plans and ensure high-quality, evidence-based care for vulnerable populations.
What you'll do

- Provide administrative oversight and support/training to case management staff.
- Maintain accurate and timely client case file documentation.
- Serve as a point of contact for program matters and external funders.

- Preferred 3 years of experience in healthcare, case management, or homeless services.
- Valid driver’s license with reliable transportation and insurance.
