Intensive Case Manager

The role

Eliot is seeking an Intensive Case Manager for their Adult Community Clinical Services (ACCS) team to provide person-centered, evidence-based interventions for adults with severe and persistent mental illness. The role involves working within a multidisciplinary team to offer community outreach, skill training, and support for personal recovery and community integration. The position requires a commitment to fostering optimism and empowerment while assisting clients in their daily lives and treatment plans.
