The role

Promote the well-being of older youth in the Permanency Services Program, participate in staff meetings and trainings, and coordinate with staff from other agencies involved in providing services.

What you'll do

  • Coordinate Independent Living and Preparation for Adult Living (PAL) services
  • Evaluate youth needs, safety, family strengths, and eligibility
  • Work with Permanency Specialists and Care Management to secure placements
  • Maintain accurate, complete case records and submit required reports
  • Monitor PAL contract service delivery and collaborate with agencies
  • Participate in case planning conferences and assist with teen events

What it takes

  • Bachelor’s or associate degree plus 2 years of relevant work experience
  • 60 accredited college credit hours plus 2 years relevant work experience
  • 90 accredited college credit hours plus 1 year of relevant work experience
  • Paid or volunteer work in social, human, or protective services
  • Cultural respect and regular attendance
  • Meet agency driving requirements
