The role
Promote the well-being of older youth in the Permanency Services Program, participate in staff meetings and trainings, and coordinate with staff from other agencies involved in providing services.
What you'll do
- Coordinate Independent Living and Preparation for Adult Living (PAL) services
- Evaluate youth needs, safety, family strengths, and eligibility
- Work with Permanency Specialists and Care Management to secure placements
- Maintain accurate, complete case records and submit required reports
- Monitor PAL contract service delivery and collaborate with agencies
- Participate in case planning conferences and assist with teen events
What it takes
- Bachelor’s or associate degree plus 2 years of relevant work experience
- 60 accredited college credit hours plus 2 years relevant work experience
- 90 accredited college credit hours plus 1 year of relevant work experience
- Paid or volunteer work in social, human, or protective services
- Cultural respect and regular attendance
