The role
Provides aftercare case management, kinship caregiver support, transitional living assistance, and parenting support services for children and youth involved in the Texas Foster Care Program.
What you'll do
- Provide up to six months of aftercare support for youth discharged from foster care
- Support kinship caregivers with verification requirements, navigating the child welfare system, and accessing services
- Assist youth transitioning to independent living by coordinating with case managers and support programs
- Deliver parenting education and support, including child development, safe sleep, childproofing, and access to community resources
- Develop and implement individualized aftercare service plans
- Maintain regular contact with children and families through in-person, virtual, and phone meetings
What it takes
- Bachelor’s degree required
- Two years of experience in child and family services preferred
- Strong communication, case management, documentation, and problem-solving skills
- Valid driver’s license and ability to meet agency driving requirements
