The role
Provides direct support to families in the Family Preservation Program to promote child safety, stability, and well-being.
What you'll do
- Provide daily direct and indirect support to families in home, school, and community settings
- Coordinate community resources (e.g., healthcare, childcare, housing, employment, benefits)
- Collaborate with case managers and therapists to implement case plans
- Offer emotional support, coaching, crisis intervention, and parenting guidance
- Assist families with accessing services such as SNAP, Medicaid, SSI, and employment support
- Maintain accurate and timely documentation and prepare records for audits
What it takes
- High School diploma or GED required
- Valid driver’s license and ability to transport clients
- Ability to promote family stability through coordinated services and engagement
