The role
Eliot is seeking Family Partners to join their Adult Community Clinical Services (ACCS) division, providing support to adults with severe and persistent mental illness.
What you'll do
- Provide in-home and community-based support to youth, adults, and families
- Collaborate with the ACCS team and families to offer support, referrals, and linkage to services
- Support parents and caregivers through advocacy, skill-building, and navigation of mental health, educational, and community systems
- Ensure all services are culturally and linguistically appropriate
- Assist families with treatment and safety planning, crisis intervention, and connecting to local, state, and federal resources
