The role
Eliot is seeking a Community Supports Case Manager to assist in completing assessments and Individual Service Plans for individuals with developmental disabilities, ensuring their involvement in goal setting and maintaining positive relationships with families and providers.
What you'll do
- Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases
- Ensure individuals are involved in identifying and developing their own goals
- Document daily progress notes for all individuals
- Develop and maintain a positive working relationship with individuals, families and providers
- Coordinate service delivery for cases assigned
What it takes
- Bachelor’s degree or prior experience in/or exposure to case management
- Experience working with intellectually disabled or brain injured adults
- Ability to manage a client caseload in an outreach setting
- Valid driver’s license and maintain a personally owned vehicle
- Ability to work community based position
