- Bachelor’s degree in Social Work, Psychology, Nursing, or a related field; OR Associate’s degree in Social Work, Psychology, or related field with at least 2 years of mental health or social services experience
- Minimum of 2 years of experience in case management, care coordination, or related social services role
- Certified Community Health Worker (or willingness to obtain certification within a designated timeframe)
