- Minimum of 2 years of experience in case management or care coordination.
- Certified Community Health Worker (or willingness to obtain).
- Bilingual or multilingual skills (Dari, Pashto) highly preferred.
- Strong cultural competency and experience with immigrant or vulnerable populations.
- Valid driver's license with comfort driving a large passenger van.
