Community Health Worker
The role
The Community Health Worker will serve as a vital member of the Substance Use Intervention Team (SUIT) at UHN, collaborating with interdisciplinary clinicians to support patients dealing with substance use. This role involves providing direct one-on-one patient support, navigating healthcare and community systems, and addressing social determinants of health such as housing and food security. The position requires a commitment to harm reduction and trauma-informed care to help patients achieve their health goals within emergency and inpatient settings.
