What you'll do

  • Build and maintain partnerships with community leaders, organizations, and businesses.
  • Collect and document program data using Salesforce and other reporting platforms.

What it takes

  • Bachelor’s degree in social work, psychology, counseling, or a related field (preferred).
  • Two years of relevant experience providing navigation or referral services if no degree is held.
  • Strong interpersonal skills with the ability to build trust across cultural differences.
  • Knowledge of social determinants of health and barriers faced by LGBTQ+ individuals.
  • Proficiency with office software including Excel, Notion, and electronic health records.
