The role
Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve’s abilities to function in the community.
What you'll do
- Provide comprehensive psychological and behavioral case management services
- Assess, monitor, and follow up with people we serve
- Ensure people we serve access to needed services and community resources
- Provide psychosocial services to improve or maintain people we serve’s abilities to function in normal social roles
- Help people we serve find and make use of appropriate employment opportunities
- Assist people we serve who need special living arrangements
What it takes
- Bachelor's Degree in psychology, social work, or other related human services
- One year experience in mental health or human services preferred
- Valid Colorado Driver’s license and a recent clean driving record
- Ability to transport people we serve in personal vehicle
