The role
PATH is seeking a dedicated Clinical Case Manager to provide field-based, direct client outreach and therapeutic services at their Bridges at Kraemer Place location. The role focuses on promoting recovery, community integration, and housing stability through individual and group therapy, case planning, and advocacy. The successful candidate will work collaboratively within a multidisciplinary team to address mental health, substance use, and functional barriers for individuals experiencing homelessness.
What you'll do
- Perform initial comprehensive biopsychosocial assessments for clients.
- Develop and maintain individualized treatment plans addressing short- and long-term goals.
- Provide field-based therapeutic support and case management services.
- Consult with clinical supervisors to conduct crisis and suicide risk assessments.
- Assist clients with applications for mainstream entitlements and housing subsidies.
