The role
To be a member of a multi-disciplined team using holistic approaches. Working within the principal of harm minimisation where residents receive a high degree of care, support and dignity.
What you'll do
- Ensure practical needs of service users are met
- Assess needs and refer to appropriate agencies
- Provide key working and advocacy for residents
- Liaise with external agencies and health boards
- Assist medical and nursing staff with health care
- Participate in supervision and appraisals
What it takes
- 2 years paid experience in social care field
- Social Studies / Case Management Accredited Qualification
- Experience and understanding of intimate partner violence
- Good working knowledge of risk assessment processes
- Understanding of harm reduction and low threshold working
- Experience with needs assessment, keyworking, and support planning
