PATH is seeking a Case Manager for their Shallow Subsidy team in Los Angeles to assist clients in breaking the cycle of homelessness. The role involves conducting intakes, developing individualized service plans, and connecting participants to essential medical, mental health, and social resources. What you'll do

  • Conduct intake and individualized needs assessments for participants.
  • Develop and monitor Individualized Service Plans (ISP) for housing and personal goals.
  • Provide referrals and linkages to medical, mental health, and vocational resources.
  • Complete program-specific assessments such as the VI-SPDAT.
  • Maintain accurate, timely, and confidential participant case files.
  • Conduct 90-day progress reviews with program participants.

What it takes

  • Two years of experience in healthcare, case management, or homeless services preferred.
  • Strong analytical, communication, and problem-solving skills.
  • Ability to maintain professional boundaries and ethical standards.
  • Must possess a valid driver’s license, reliable transportation, and proof of insurance.
  • Ability to successfully complete background screening, drug test, and TB test.
  • Willingness to obtain CPR/First Aid training.
