Case Manager

The role

Eliot is seeking a dedicated Case Manager for their Program for Assertive Community Treatment (PACT) to provide behavioral health support to adults with mental illness. The role involves managing individualized treatment plans, conducting clinical assessments, and building therapeutic relationships within the community. The position requires a multidisciplinary approach and participation in rotating on-call and evening shift coverage. You will join a mission-driven team focused on empowerment, resilience, and clinical excellence.
