What you'll do
- Conduct intake and assessments
- Provide case management and counseling services
- Assist clients in creating Individual Living Plans
- Input client information into CARES system
- Collaborate with clients and staff to identify financial entitlements, housing, and other concrete services
- Maintain accurate written documentation
What it takes
- High School/GED Diploma or Associate's Degree
- One to two years of experience working with homeless adults, substance use and/or mental health populations
- Teamwork, Organizational, Interpersonal, Communication, Computer skills
- Bilingual a plus
- Knowledge of case management software as well as proficiency in Microsoft Office Suite
