The role
PATH is seeking candidates to join their Permanent Supportive Housing Team as a Case Manager for their new Alma Apartments site in Los Angeles, CA. The role involves assisting clients in breaking the cycle of homelessness through intensive case management services.
What you'll do
- Conducts intake and individualized needs assessment for all clients
- Collaborates to develop Individualized Service Plan (ISP)
- Provides referrals, linkages, and support to resources
- Completes program specific assessments
- Evaluates strengths and challenges to addressing goals
- Maintains documentation standards
