The role
MHA is seeking a full-time Case Manager to join their Prospect House Team, responsible for providing partial care services and case management to individuals with severe and persistent mental illness.
What you'll do
- Provide partial care services to individuals with severe and persistent mental illness
- Plan and facilitate therapy, coping, activity and psycho-educational groups
- Provide case management services for members
- Maintain electronic clinical records
- Collaborate with Program Director and Unit Leader to develop program
What it takes
- Bachelor's degree in a human services field, or an Associate's degree and two years’ experience in providing human services, or five years of human service experience
- Certified Psychiatric Rehabilitation Practitioner, Certified Alcohol and Drug Counselor, or other relevant professional credential
- Computer literacy
- Experience in case management, crisis intervention, and record keeping
