The role
The Case Manager at Launch Housing supports clients experiencing homelessness through a range of intervention options and case management strategies.
What you'll do
- Engage with clients to determine eligibility for services and refer them to other relevant services if needed
- Develop case plans in conjunction with clients and implement strategies that enable clients to achieve their goals
- Assist clients in developing longer-term housing options both within and independently of Launch Housing's supported accommodation program
- Ensure clients maximize all government and community income assistance and support
- Initiate and maintain contact with vulnerable clients using flexible engagement strategies
- Coordinate services required by the client and monitor their ongoing effectiveness

- Relevant tertiary qualification in human services
- Direct experience in delivering services to clients with complex needs related to homelessness
- Understanding of housing and homelessness policy, context, and systems
