The role
CAMBA is seeking a Case Manager to work with chronically homeless individuals with serious mental health illnesses in the Highbridge Overlook Supportive Housing Program in the Bronx, NY.
What you'll do
- Maintain professional relationships with clients and compliance with HIPAA regulations
- Conduct initial intake and assessment of tenants and tenants' families' needs
- Assist tenants in attaining their goals by identifying and locating community resources
- Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves
- Conduct group sessions and/or workshops to assist tenants with independent living skills
- Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits
What it takes
- Bachelor’s degree (B.A.) and/or equivalent experience
- Experience working with the Seriously and Persistently Mentally Ill (SPMI) population
- Ability to maintain fingerprint clearance
- Mandated Reporter experience
- Experience in case management and tenant advocacy
- Proficiency in database management
