The role

Easterseals MORC is seeking a Case Management Coordinator for their Community Outpatient program in Auburn Hills, MI. This role involves assessing the needs of individuals, developing strength-based treatment plans, and coordinating community-based services. The coordinator will provide advocacy and support while working with individuals to achieve their recovery goals.

What you'll do

  • Assess and evaluate individual needs to develop strength-based, person-centered treatment plans.
  • Screen for co-occurring disorders and identify appropriate substance abuse or mental health interventions.
  • Maintain ongoing caseload contact and document all clinical and legal interactions.
  • Advocate for client needs and develop community resources to support treatment goals.
  • Collaborate with community partners to facilitate referrals and service coordination.

What it takes

  • Bachelor's degree in Human Services, Psychology, Social Work, or a related field.
  • Must be a designated CMHP in accordance with Medicaid Provider Manual Guidelines.
  • Experience in examination, evaluation, and treatment of minors and their families.
  • Valid Driver's License and auto insurance.
  • Relevant clinical licensure (LLBSW, LBSW, LMSW, LPC, or LLP) preferred depending on experience level.
