Easterseals MORC is hiring a Case Management Coordinator to assist in making a difference and joining a team focused on helping others.
What you'll do
- Engage individuals in a welcoming, hopeful, and empathic manner
- Screen for co-occurring disorders and recognize diagnostic criteria
- Assess and evaluate the needs of individuals
- Develop goals and person-centered plans
- Complete referral forms and clinical documentation
- Collaborate with other SA/MH community partners
What it takes
- QMHP in accordance with Medicaid Provider Manual Guidelines
- Specialized training or one year of experience in treating or working with individuals with mental illness
- Bachelor’s degree in a human services field
- Experience in engaging individuals in a welcoming, hopeful, and empathic manner
- Ability to screen for co-occurring disorders and recognize diagnostic criteria
- Experience in assessing and evaluating the needs of individuals
