What you'll do
- Conduct comprehensive in-person assessments of physical and social/emotional functioning.
- Develop and implement care plans in cooperation with participants and their representatives.
- Secure necessary physician approvals and ensure documentation meets state electronic data system requirements.
- Arrange and purchase health and social services as outlined in the care plan.
- Perform standardized re-examinations of client status every 90 to 180 days.
What it takes
- Bachelor’s degree in Social Work from an accredited college or university.
- Must be eligible for or currently hold an LLBSW or LBSW license.
- Possess a valid Michigan driver's license.
- Experience in case management or MI Choice waiver preferred.
