PATH is seeking a dedicated CalAIM Case Manager to provide essential support to Medi-Cal beneficiaries experiencing homelessness in San Diego, CA. What you'll do
- Conduct intake and develop Individualized Service Plans (ISP) for participants.

- Complete program-specific assessments such as the VI-SPDAT.
- Maintain accurate documentation and timely case file notes for all participants.
- Evaluate participant progress and conduct 90-day review meetings.
What it takes
- Two years of experience in healthcare, case management, or homeless services preferred.
- Valid driver’s license, reliable transportation, and proof of insurance required.
- Ability to pass a background screening, drug test, and tuberculosis test.
- Must be authorized to work in the U.S. without visa sponsorship.
- Ability to obtain CPR/First Aid training.
