- Conduct 90-day progress reviews with clients.

- Two years of experience in healthcare, case management, or homeless services preferred.
- Must possess a valid driver's license, reliable transportation, and proof of insurance.
- Ability to qualify for PATH's insurance coverage for driving.
- Willingness to obtain CPR/First Aid training.
- Must successfully pass background screening, drug test, and tuberculosis test.
